Duties:
Program Managers have the primary responsibility of overseeing, directing and supervising staff to fulfill the physical and emotional well-being of the Hearthstone member(s) in their care during the agreed upon hours of employment, in accordance with Hearthstone's goals and Gentle Teaching philosophy, accountable to and following any specific instructions given by the General Manager.
Experience
- 1 year previous experience in providing long term care or chronic care to individuals /residents is preferred.
Qualifications and Skills
- Knowledge of basic nursing care principles, techniques and procedures.
- Knowledge of Personal Health Information Act (PHIA), Workplace Hazardous Material Information System (WHMIS), Principles of Routine Practices and other relevant Workplace Safety and Health requirements/responsibilities.
- Skill in providing specialized and/or long term nursing care to patient/residents.
- Skill in application of appropriate techniques for patient/resident lifting, transferring and mobilization.
- Basic computer and keyboarding skills.
- Ability to express ideas and thoughts in English both verbally and in written formats.
- Ability to select and use appropriate communication methods pertinent to the situation.
- Ability to organize workload and set priorities.
- Maintain confidentiality.
- Ability to establish and maintain positive interpersonal relationships with Individuals , family members and coworkers.
- Accountable to and following any specific instructions given by the General Manager.
- Ensure that our individuals have the appropriate physical and emotional supports that ensure growth, interdependence, and the well-being of those participating in Day Program. Seek out feedback and listen to their concerns.
- Model and promote Gentle Teaching Principles. Intervene and coach staff to interact appropriately and safely with our individuals.
- Model and ensure application of the principles of the Vulnerable Persons’ Act. Report any indications of abuse (See Prevention and Management of Abuse Policy).
- Create personalized Program Plans for each individual based on the Service Definitions of Building on Abilities. Create and ensure documentation on goals for each person. Employ person centered planning (IPP, Paths). Make recommendations for staff support. Participate in other plans (i.e., Safety Plans, SIS, Diet Plans).
- Research and implement creative program activities.
- Liaise with the families and Individuals of Day Program participants; attend planning and intake meetings, note concerns and facilitate communications.
- Ensure accurate program attendance records
- Provide appropriate meal planning, menus and special diets where applicable in conjunction with dieticians and/or staff.
- Oversee medication administration. Ensure staff are trained for specialized medical needs as required.
- Complete new hire orientation plans: schedule and follow up on required training.
- Create staff schedules and work assignments. Monitor time off and schedule changes.
Note: This is a term position, successful candidate may transition into House Manager position after term.
