WORKING CONDITIONS
JOB DUTIES AND RESPONSIBILITIES
- Develop culturally-based safety, wellbeing and supportive programs geared towards helping young adults formerly in care.
- Coordinate family reconnection of out of care young adults.
- Develop support programs that would aid the out of care young adult to easily access culturally appropriate supports and services that meet their needs.
- Implement the Blackfoot culture into programming.
- Assist clients in goal setting based on individual needs assessments.
- Identify steps to achieve client goals and monitor progress and assist in removing barriers to individual success.
- Work with the young adults formerly in care to be connected and actively engaged in their community, language and culture.
- Provide support to clients including advocacy, referral, transportation, and mentoring.
- Collect and document data to track client successes.
- Facilitate group presentations based on trends of clients needs.
- Develop programs based on available funding to support stability and connections in the areas of housing, employment, education, mental health and wellbeing.
- Conduct quality home visits.
- Complete appropriate paperwork for program reporting and interoffice communication.
- Coordinate, schedule and attend case conferences and appointments with community providers with whom the client is involved.
- Maintain regular contact with the clients network of service providers.
- Participate in weekly staff meetings with the management team to evaluate clients and program progress.
- Attend and participate in weekly individual supervision meetings with the Executive Director to discuss client needs, goals and challenges.
- Adhere to all PCFS policies and procedures.
- Maintain confidentiality and professionalism.
- Other duties as assigned
- Degree in Social Work, Early Childhood Education or Human Services. Other related education will be considered.
- At least two years experience in the human services field, or a related field.
- In depth understanding of community resources in Piikani and surrounding areas.
- Blackfoot language and culture knowledge is preferred.
- Strong leadership and supervisory skills, with the ability to motivate and support a team.
- Knowledge of resources in early childhood and parenting programs.
- Clean Criminal, and Child Intervention Record Check.
- Drivers license and reliable transportation. (Drivers abstract required)
- Excellent organizational and time management skills.
- Must be able to work evenings and weekends if required.
- Experience dealing with confidential and sensitive information.
- Effective communication skills, both written and verbal.
- Display kindness, empathy and compassion to families.
