The Oxford County Community Health Centre (OCCHC) exists to address the needs of community members who face a range of barriers in everyday life.
We are seeking:
TRANSITIONAL HOUSING WORKER – TILLSONBURG
The Transitional Housing Worker (THW) supports individuals experiencing homelessness in Oxford County, specifically Tillsonburg and area, through our Transitional Housing program. The transitional living residence – known as the Light House – is an inviting 7 bedroom home proudly owned by the United Way Oxford, with expectations that the OCCHC delivers the highest quality transitional living program in line with best practices.
Overview – the incumbent will:
- Receive referrals / meet with prospective Transitional Housing clients to complete intake procedures – assess & triage client needs, & determine eligibility for transitional living.
- Engage in outreach interactions with the unhoused population by attending the Tillsonburg Shelter, and Day Space (forthcoming), on a regular basis.
- Assist Light House clients to develop basic life skills through coaching & goal planning; building motivation & confidence to achieve goals & stability; instilling a sense of emotional & social belonging; developing creative group programs; & establishing trusting relationships.
- Uphold daily operations e.g. guest guidelines, health & safety, room checks, individual client support to attend appointments, house seeking, maintenance, conflict mediation, discharges & move-in’s, & household basic supplies.
- Build strong & productive relationships with community partners & landlords.
- Navigate community resources, & transition toward permanent housing.
Compensation: 24 month contract with possibility of extension. Competitive compensation & in-lieu % benefits.
Hours of Work: 37.5 hours weekly - Monday to Friday 8:30 a.m. – 4:30 p.m.
Requirements:
- Completed post-secondary education in social services or related field and a current Member of a regulatory college in good standing, such as the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
- Considerable experience addressing the needs of individuals and families experiencing homelessness, or those at risk of homelessness.
- Understanding of community agencies and a wide cross-section of service providers – preference will be given to candidates with demonstrated knowledge of Tillsonburg & Area communities.
