Community Outreach & Prevention Worker

Job Purpose

The Community Outreach & Prevention Worker is responsible for providing wholistic, non-judgmental care for the nation’s most vulnerable members. The Community Outreach & Prevention worker will provide crisis response in the event of an emergency and will possess some medical or first aid background as well knowledge of mental health and suicide intervention practices. They would be available to provide education and support, follow up care when needed and increased access to other services within the region.

Duties and Responsibilities

  • Providing assertive or planned outreach services as required by the client.
  • One on one contact with community members; Engages with each client to facilitate a supportive working relationship.
  • Assist in accessing health, housing and social services and remaining involved until the necessary linkages are met
  • Assist in enhancing life skills and promoting healthy lifestyle activities.
  • Advocating on behalf of clients and/or supporting them to advocate for themselves in accessing resources that contribute positively to the social determinants of health.
  • Respond to crises as they arise within the community from a harm reduction perspective.
  • Collaboration & coordination of services with appropriate community agencies.
  • Supporting and facilitating clients’ use of appropriate community resources by keeping knowledgeable about available services in the region.
  • Liaising with hospitals and community agencies.
  • Collaborates with members of the Family Support Services Team to coordinate, participate, and assist with family related activities within the community.
  • Being aware of and responding to risk indicators that identify a need for intervention (medical, clinical, legal, etc.) to ensure the safety of an individual or others.
  • Documents progress notes which may include digital records.
  • Identifies gaps in service delivery and plans programs accordingly.
  • Participates in training and educational programs as required.
  • Participates actively as a SFN representative in collaborative partner networks and related community activities.
  • Performs other related duties as directed by Manager.

Required Qualifications

  • Preference will be given to candidates who possess the following certifications, however training will be provided as needed:

* First Aid – OFA Level 3

* Mental Health First Aid

* Cultural Competency Training

* Crisis intervention training

* Working with victims of Trauma

* Suicide prevention training

* Selfcare training

* Addictions & Substance use Training

  • Post-Secondary education in Social or Human Services is a preference but not required.
  • Ability to pass a Vulnerable Sector Criminal Records Check
  • Valid BC Class 5 driver’s license supported with a clean driver’s abstract

Required Knowledge, Skills and Abilities

  • Ability to analyze problems and develop effective solutions
  • Knowledge and understanding of First Nations issues, culture, and traditions.
  • Awareness and understanding around the impact from trauma and inter-generational trauma resulting from colonial policies.
  • Ability to respond to problematic behavior using an understanding of the behavior’s context and function.
  • Excellent organizational, analytical, and problem-solving skills.
  • Excellent computer skills including MS word, Outlook and Excel.
  • Superior conflict resolution skills and ability to effectively engage with diverse stakeholders as well as community members.
  • Excellent verbal, written, and interpersonal skills.
  • Must practice and display a high standard of work ethic specifically maintaining confidentiality.
  • Strong community engagement skills and the ability to build partnerships.
  • Physically able to stand for extended periods of time.
  • Ability to lift up to 50lbs.
  • Basic sound knowledge of mental health diagnoses, medications, treatment, and support resources.
  • Basic knowledge of Harm Reduction theories, programs and supplies.
  • You are passionate about the Organization’s mission and understand the current and historical context of the child welfare system in relation to colonization.

Work Conditions

  • This role requires a flexible work schedule based on programming and community-based support services.
  • Some work may be performed during Saulteau’s regular office hours; however, some programming will fall outside of the regular office hours.
  • Travel will be required within SFN community and surrounding community.
  • Some evening and weekend work will be necessary.
  • Departmental vehicle supplied.

Job Types: Full-time, Permanent

Salary: $21.60-$27.04 per hour

Expected hours: 32 per week
