Job Description: The quarter-time Health Educator is responsible for the overall planning, development and implementation of the Live Well with Chronic Conditions (LWCC) program. This includes defining program goals, developing program strategies, marketing and recruitment of program participants and volunteer facilitators, identification of deliverables, timing and overall coordination of project activities. The Health Educator will develop registration processes, implement data collection and analysis of program results, develop and coordinate a volunteer base (including on-going support and training), work collaboratively with the LWCC team on multiple levels, establish communication plans, and identify risks and contingency plans. Specific to the Stanford Chronic Disease Self-Management Program, the Health Educator will ensure the quality of the program as per training and licensing agreements. A significant component of the program involves contact and a good working relationship with non-governmental and community groups. This position is responsible for the LWLCC program for both urban and rural areas in PAPHR, so some travel is required. This position requires the use of field hours, i.e. some evening work for program coordination, promotion and marketing may occasionally be required.
Experience
QUALIFICATIONS
Requires a minimum of a four years bachelors degree in one of the following disciplines: Health Science, Social Science, Nutrition, Occupational Therapy or Physical Therapy.
KNOWLEDGE, SKILLS & ABILITIES
Preference would be given to candidates with demonstrated understanding of and experience in principles of adult education, facilitation and project management.
Prefers candidates with Live Well with Chronic Disease Master Trainer Designation
