Evening Tuesday to Saturday 3:00 pm to 11:00 pm
Overnight Tuesday to Saturday 11:00 pm to 7:00 am (two week contract)
Casual Friday to Saturday 7:00 am to 3:00 pm
Casual Sunday to Monday 3:00 pm to 11:00 pm
Casual Sunday to Monday 11:00 pm to 7:00 am (two week contract)
Main Purpose of the Job:
The purpose of this position is to provide in-Home Support to an individual with FASD in a manner that maximizes their independence, potential and quality of life.
Duties and Responsibilities:
· Support client(s) to be as engaged and independent as possible in daily living, work, vocation, leisure, and recreational activities
· Work collaboratively with client(s), co-workers, team members, families, the community, and other organizations to implement goals
· Assist, respect, and support client(s) to make choices and decisions, using supportive decision-making practices.
· Assist client(s) with goal setting and support planning in a variety of areas.
· Communicate effectively with client(s) using a variety of techniques which may include pictures, tactile prompts, and sign language.
· Understand and use positive behaviour support techniques
· Identify triggers and address potential harmful/dangerous behaviour preventatively or as it arises using respectful intervention techniques
· Assist with personal care according to each person’s needs (this may include basic routine care, bathing and personal hygiene, support with eating, or an other tasks necessary to promote people’s well-being)
· Assist client(s) with medication and specialized procedures as required and in accordance with established medication policies and procedures
· Provide physical assistance to client(s) with mobility limitations where required (this may include lifts, transfers, and repositioning).
· Support in meal preparation and model housekeeping duties
· Provide or facilitate transportation for client(s) when necessary (such as to work, appointments, community events, etc.)
· Support people with skill development in a variety of areas
· Act as a positive role model. Encourage positive interpersonal relationships, and coach/motivate/support client(s) to build life and social skills
Other Responsibilities
· Maintain records, ensuring confidentiality and accurate documentation of individual activities, general health, progress, skill development, support needs, medications, specialized procedures, finances, behaviour and incidents
· Provide written and oral progress reports, summaries and recommendations on individual goals.
· Monitor for and report any safety hazards
· Track supplies and submit requests for resources needed
· Participate in meetings such as staff, service delivery, training, etc.
· Other duties may be assigned.
Qualifications:
· High school or equivalent
· Disability Support Worker Certificate or equivalent education
· A satisfactory Criminal Record Check with a Vulnerable Sector Check
· Proficient in spoken and written English
· Valid Driver’s License and access to a reliable vehicle
· Knowledge of FASD is considered an asset
· Ability to work independently
Job Type: Contract
Salary: $23.00 per hour
Expected hours: 40 per week
