A division of the Population Health and Value Based Health Systems portfolio, the Ontario Health Teams Strategy & Policy portfolio works closely with OH regions and OHTs to support the effective implementation of programs and initiatives to advance population health and integrated care. The Lead, OHT Implementation is responsible for leading multi-faceted and innovative projects and initiatives to advance OHTs across the province.
The Lead, OHT Implementation will focus on ensuring the availability of and access to a coordinated program of implementation supports aligned to the needs of Ontario Health Teams. The Lead works collaboratively with broad internal and external stakeholder groups to design, integrate and monitor efficacy of program implementation. The Lead may be asked to deliver Ontario Health Team implementation supports by leveraging subject matter expertise in change management, clinical pathways, organization development, project management, knowledge translation, quality improvement and implementation science. The Lead has strong service orientation, is resourceful(?) and curious and thrives in a fast-paced, dynamic environment.
Here is what you will be doing:
- Oversees day-to-day Ontario Health Team program operations and implements assigned initiatives.
- Defines program / operation scope and deliverables in coordination with Manager / Director, with a focus on providing input on area(s) of expertise
- Applies knowledge of complex change management, defines technical and adaptive challenges and designs and recommends solutions to advance strategic priorities
- Leads forums for knowledge exchange and information sharing amongst a broad range of internal and external stakeholders, integrating feedback and data to inform program design and delivery
- Works with Ministry, OH provincial portfolios, OH regions and internal and external support partners to build consensus and coordinate efforts to address complex system initiatives (e.g regional delivery models, equity planning, etc.)
- Provides coaching and advice to diverse stakeholder groups for effective and efficient processes to engage and communicate with Ontario Health Teams
- Identifies and synthesizes resources and information from multiple sources to co-design implementation strategies, assets and resources
- Acts as conduit/broker and subject matter expert, understanding stakeholder strengths, needs and barriers to inform regional and provincial planning and facilitate effective feedback processes
- Regularly assesses alignment and efficacy of implementation supports, based on changing needs
- Builds consensus through leadership of and participation in provincial and regional working committees and stakeholder engagement sessions
- Interprets data to identify gaps and make recommendations to mitigate risk and achieve desired outcomes
- Identifies and works with others to resolve issues related to alignment of programs to principles of population health management and health equity
Here is what you will need to be successful:
Education and Experience
- Undergraduate degree in relevant discipline is essential; graduate degree (e.g. MBA, MA, MHSc, MHA, MPA) would be highly advantageous.
- 5-7+ years’ experience in health care program design and delivery, stakeholder engagement or related role; relevant experience in the healthcare sector a strong advantage.
- Experience and formal training combined with demonstrated performance and ability may substitute for stipulated.
- Experience with stakeholder engagement including effective application of collaborative tools / platforms (e.g MS Teams, Zoom, SharePoint) required
- Fluency in French a strong asset
Knowledge and Skills
- Strong service orientation with demonstrated relationship-building skills
- Must be team-oriented, possess a positive attitude and work well with others
- Knowledge of the Ontario healthcare system and key system priorities, including the Ontario Health Teams strategy
- Extensive knowledge of healthcare sectors with experience working within a variety of integrated health care networks / models is an asset
- Understanding of the province’s vision for transformation of the healthcare system and the role of Ontario Health Teams in planning and delivery of integrated health and wellness care at the local level
- Experience working with senior level health system leaders and other key stakeholders on complex issues with the goal to develop local / regional health workforce solutions
- Proven knowledge of best practices, evidence and tools to support integrated care, population health, health equity and transformative change
- Experience leading complex strategic projects
- Strong business acumen, leadership and critical thinking abilities
- Ability to develop and deliver presentations confidently to a variety of audiences
- Completion of diversity / cultural sensitivity training
- Superior interpersonal, communication skills and ability to successfully build and maintain relationships
- Demonstrated analytical and problem-solving skills
- Ability to facilitate productive discussions among diverse partners
- Ability to work effectively in both a team setting and independently
- Ability to analyze and interpret healthcare and population data/information
- Strong written and verbal communication and presentation skills with an ability to translate complex ideas and concepts for both executive and public audiences
