Resident Assessment Instrument-Minimum Data Set Registered Practical Nurse

WAGE RANGE: $39.19 - $40.41 per hour (40 hours / week)
POSITION SUMMARY:
Reporting to the RAI RN and in collaboration with the Coordinator of RAI, is responsible for accurately completing all resident RAI-MDS 2.0 assessments, Resident Assessment Protocols (RAPs), resident care plans, and other required nursing assessments and duties as assigned.
KEY ACCOUNTABILITIES:
1. Accountable for own actions to the College of Nurses.
2. Follow all Practitioners orders, receiving and transcribing verbal and written orders.
3. Complete accurate and detailed RAI assessments, RAP’s and care plans as well as the Nursing Summary and other quarterly nursing assessments by deadlines according to the RAI weekly schedule.
4. Prioritize documentation workload in order to meet RAI timelines and deadlines.
5. Maintain up to date care guides for assigned residents according to the RAI weekly schedule.
6. Communicate with interdisciplinary team, residents, and their families to acquire in-depth knowledge for completing assessments and make recommendations to improve resident quality of life.
7. Work in collaboration with the Coordinator of RAI on new initiatives or projects within the facility as required.
8. Provide support for the RAI RN and the Coordinator of RAI in their absence, including, but not limited to, data submission, scheduling, and communicating with inter-disciplinary teams.
9. Provide education and support to interdisciplinary teams, residents, and families within the long-term care home.
10. Provide person- and family-centered care to support physical, emotional, social, and spiritual needs of residents.
11. Monitor quarterly Resource Utilization Group (RUG) trends in attempt to maximize Case Mix Index (CMI).
12. Audit and ensure Point of Care (Point Click Care) documentation accuracy and present findings to RAI RN and RAI Coordinator as required.
13. In collaboration with the restorative care designated lead, ensures that restorative care approaches are integrated into the care that is provided to all residents.
14. Performs other duties as assigned.
QUALIFICATIONS:
1. Current registration with the College of Nurses of Ontario in good standing
2. Extensive knowledge of Point Click Care documentation system
3. Experience with MDS coding and RAPs
4. Experience in creating and updating Care Plans within Point Click Care
5. Well developed oral and written communication skills
6. Clear aptitude for detail-oriented work, ability to work with and relate to interdisciplinary team members and other internal stakeholders
7. Proficient computer skills
8. Successfully complete mandatory annual RAI-MDS proficiency evaluations
9. Successful completion of RAI-MDS 2.0 education aimed at learning how to successfully use interRAI Worldwide instruments for long-term care facilities.
