Job description

Position: Rehabilitative Care Worker, The Meadows ARC, Bridgetown, NS.

Full Time, Term (13 months) with Medical, Dental Benefits and Pension

Start Date: As soon as possible

Position Scope:

RCWs provide a variety of supports for people with intellectual disabilities and long-term mental health challenges. They are responsible to the LPN Manager for all aspects of program development, implementation and evaluation, the supervision, and the daily support of the clients, and the maintenance of their well being.

In the absence of the Manager, duties are performed under the directional framework as set out by the Director of Disability Support Programs.

Key Areas of Responsibility:

Maintain supportive, professional, client centered relationships that are inclusive and fosters independence. Develop and maintain effective relationships with families, volunteers, the community, and other stakeholders.

To assist participants in learning skills necessary for activities if daily living in accordance with each client’s goals, objectives, and support plan.

Assist participants to learn and develop skills of interest. Develop, write, implement, and evaluate individual support plans, in consultation with the clients, and, under the supervision of the LPN Manager. Complete assessments, support plans and other reports as directed.

To maintain daily and monthly records of participant care, goals, condition, and progress

Dispense medications as ordered by the Physician, or, where appropriate, assist, teach or monitor clients in self administration of medication and recording of same. Give or supervise any treatments ordered by the Physician, and as directed by the LPN Manager.

Assist and instruct clients to learn nutritional menu planning, meal preparation and grocery shopping. Support and assist, or do food preparation, as appropriate. Support will vary on the individual participant support needs. Follow special dietary requirements as directed by consulting Dietician.

Maintain household cleanliness and a safe environment of our participants and staff, including housekeeping and laundry duties. Assist and support participants who are capable to assume responsibility for keeping their home safe and clean. The degree of assistance provided will vary depending on the skill level of the participant.

To attend and participate in house meetings, support plans, and in service education for the purpose of maintaining and upgrading skills and knowledge.

To maintain appliances and equipment in a clean and safe operating condition. i.e. refrigerator defrosted and cleaned, stoves, washers and dryers, clean and in good working condition. Report anything of a concern.

Participate and/or assist in recreational events, in house and in community.

Transport clients to activities and events as required, with mileage payable at prevailing rates or use company van.

Assist/support clients to learn money management and budgeting skills. Keep accurate written records.

To be aware of safety hazards in the home and WHIMIS regulations.

To apply supportive decision making to our participants when needed

Judgement and Initiative:

Must be flexible in dealing with changing or challenging situations, supports the needs of participants, presenting an attitude of respect, empathy and understanding.

Must be a positive role model, encouraging good morale, demonstrating respect, and providing advocacy as needed.

Personal integrity, initiative and good judgement are required.

The ability to cooperate with other staff and work harmoniously as a member of a team is essential.

Physical Demands:

Regular housekeeping, cooking, and housecleaning tasks. Recreational events. Must be physically and emotionally able to fulfill job requirements, including assisting clients with ambulation and personal care. Standing, walking, lifting, and working the night shift are involved.

Working Environment:

Much of the work is in the small option home. Some travel is included such as some outside activities, events and appointments.

Hazards:

While working with clients, may, from time to time, be exposed to behaviours such as physical and verbal abuse. Cuts, scrapes, chemical burns, cigarette smoke, or injuries, may result from routine housekeeping tasks. Pets and plants are part of the normal working environment.

General:

Responsible to inform the LPN manager of any incident where staff or clients are injured or in need of assistance, or where a major behavioural problem occurs.

Responsible to record daily all significant events and incidents in appropriate logs and to record programs.

Complete any incident reports and report incidents of abuse to LPN Manager

Carry out fire, safety and emergency procedures without supervision, teach and reinforce client education regarding fire safety and emergency procedures.

Responsible to promptly report all accidents, incidents, and illness to LPN Manager

Encourage good client morale, relate to clients in an age-appropriate manner, and maintain an atmosphere that is warm, positive, homelike and respectful of clients’ needs and rights

Must have a valid Nova Scotia driver’s license, insurance as required and be willing to transport clients using own vehicle. Must obtain vulnerable sectors check from RCMP.

Must maintain acceptable personal hygiene and grooming.

Qualifications:

High school diploma

Successful completion of the Disability Supports Program, Human Services 2-year diploma program and / or equivalent.

Attainment of Core competencies as per Provincial standard for Rehabilitative Care Workers funded under DCS.

Current Crisis Prevention Intervention certificate

Certification in Standard First Aid and CPR

Food Handlers

WHIMIS

Recent related job experience, including the development and implementation of individual support plans.

Special Knowledge and Skills:

Must have a sound knowledge of and be able to practice the principles of diversity and inclusion. Empathy with clients, warmth, patience, imagination, teaching ability, creativity, and a positive approach toward community integration, are all important requirements.

Must be willing to attend in service education, seminars, workshops, other educational events, and participate in team meetings

Knowledge of Supportive Decision Making and the UN convention of Human Rights are a must.

Please note this is a CUPE position and salary is per the collective agreement.
