The Algonquin Nursing Home is a bilingual 73-bed long-term care facility located in the town of Mattawa, Ontario, and is sponsored by the Catholic Health Sponsors of Ontario, licensed under the Ministry of Health, and accredited by Accreditation Canada. The Home’s goal is to use a team approach in order to provide a safe, clean, comfortable and pleasing home atmosphere for residents and their families.
We are seeking a highly motivated Resident and Family Services Coordinator with excellent communication skills to join its leadership team. This resident-centred position has a strong focus on driving the vision, mission, and values of the Home by putting the resident experience at the forefront of all decision-making.
Primary Duties:
- Manage the delivery of an organized program of recreational and social activities for the Home to meet the interests and needs of the residents;
- Facilitate relationships between and among residents, relatives, staff and community;
- Recruit, orientate, train, evaluate, supervise, and engage staff, students and volunteers;
- Coordinate resident intake, transfer, and discharge in consultation with stakeholders;
- Act as a support to residents and their families, including recommending appropriate resources for crisis and difficult situations and when supportive intervention is required;
- Coordinate the restorative, palliative, and spiritual care programs with an integrated approach;
- Assist with the provision of a resident advocacy program through the Resident’s Bill of Rights;
- Develop, review, and evaluate policies and procedures and best practices for resident care in collaboration with other departments;
- Ensure fiscal responsibility with approved budget in program delivery;
- Participate on committees, as assigned;
- Participate in After-Hours Management on Call;
- Other duties, as required.
Mandatory Requirements for the Designated Lead – Recreation and Social Activities Program, as described in Fixing Long-Term Care Act, 2021and Regulation 246/22:
- Experience in a health care setting or other relevant setting, and
- Post-secondary diploma, degree, or certificate in recreation and leisure studies, therapeutic recreation, kinesiology, gerontology or other related field from an Ontario post-secondary institution.
Additional Qualifications:
- Minimum two (2) years of experience in managing a comparable resident and family service
- Specialized knowledge of Gerontology and Palliative Care
- Experience with Point of Care (preferred)
- Bilingual in English and French (preferred)
- Fully vaccinated against Covid-19 (subject to accommodation obligations pursuant to Ontario Human Rights Code)
Salary Range: $50,992 to $63,745 per year (37.5-hour work week), plus comprehensive health/dental benefits and pension.
