An integral member of the multi-disciplinary Public Health team. Provides culturally appropriate liaison, counseling, case
management and health system navigation services for clients. Plans, organizes, assesses and evaluates clinical and
psychosocial health care needs of clients for referral to appropriate services. Facilitates connections between clients,
families, service providers and key local/regional stakeholders to enhance access to the Northern Health Region’s
services within a culturally appropriate context to further the health and well-being of the region’s population.
QUALIFICATIONS
- Eligible to practice and maintain membership with the Manitoba College of Social Workers
- Valid Manitoba Class V driver’s license and access to a vehicle
- Thorough knowledge of Windows based programs (Microsoft Word, Excel, Outlook) and Internet
- Two (2) years clinical social work experience in a hospital or community setting
- Satisfactory employment record required
- Successful candidate subject to provide security checks in accordance with Security Checks for Employment
