Home Family Support Coordinator
We are looking for an experienced, efficient, and responsible coordinator to provide direct supervision for a team of in- home family support workers from a person-centered approach to care and delivery of care. The team of support workers provide care and support to an engaging and energetic 37-year-old man within his family home. The home family Support Coordinator will also liaise with the family as part of a wraparound approach to ensure that the quality and delivery of care is meeting the needs of their loved one and aligns with the family’s values. Below is a summary list of qualifications and responsibilities for this position:
Key Responsibilities:
· Provide oversight and supervision (as needed) to family support workers including coaching, feedback, performance assessments and performance management. The coordinator is also responsible for scheduling.
· Assist the family with job posting, interviewing, hiring, and firing of supporters as needed.
· Provide orientation and training of new supporters and maintain and update protocols.
· Provide supporters with updates and current information regarding medication, diet, health, routines, protocols, best practice's, and personal care needs of the person.
· Conduct regular team meetings and check ins.
Knowledge, Skills & Abilities:
· Ability to practice person centered care –knowledge of Social Role Valorization theory is an asset.
· Demonstrated problem solving, crisis /risk management and intervention skills are required.
· Proven expertise in respectfully collaborating and problem solving with teams.
· Excellent communication skills including writing, active listening, and observation skills.
· Demonstrated time management and attention to detail.
· Identifies as a lifelong learner with the ability to practise humility and self-reflection to better understand others’ personal experience.
· Proven experience working with people with a disability and their families is an asset.
Requirements:
· While a medical related degree is not a requirement, a medical background, knowledge and/or work experience in the medical field is an asset.
· A driver’s license and access to a vehicle or a reliable means of transportation is required to attend meetings and perform regular check ins.
· As a condition of employment, applicants will be required to provide a current and satisfactory criminal records check including a vulnerable sector search.
Job Type: Fixed term contract
· Contract length- 5 months (November 1st, 2023- March 31st, 2024) with the potential to renew.
· Rate of pay- $25 and hour, 6hrs per week (some of which can be done remotely)
