Community Care City of Kawartha Lakes is a health and community support agency which provides a range of services designed to promote health for all ages and independence of seniors and special needs clients. We are currently accepting applications for a Personal Support Services (PSS) Supervisor based in Lindsay, ON to contribute to the PSS supervision team for Supportive Housing Services, Assisted Living for High Risk Seniors, In-Home Respite, and other PSS programs as required.
KEY ACCOUNTABILITIES:
Reporting to the Program Manager, Personal Support Services:
Undertake client assessments, client care and service plans, attending to all related documentation to ensure quality service and effective risk management
Supervise service staff, including performance review and development
Coordinate requests and manage waitlists
Ensure Personal Support Services are delivered in a safe and effective manner
Contribute to PSW/client scheduling and coverage for quality service delivery
Assist in the co-ordination, recruitment, selection, orientation, training for Personal Support staff
Provide in-service training to PSWs on delegated health services, client-specific training needs and professional development
Coordinate, prepare and maintain all related documentation for the effective operation of Personal Support Services in a timely and thorough manner
Contribute to team effectiveness and success of all Personal Support Services programs
Provide coverage for fellow PSS Supervisors
Share after-hours on-call responsibility with other PSS Team members
KEY COMPETENCIES REQUIRED FOR THIS POSITION
Registered Practical Nurse
Care coordination experience
Strong documentation and assessment skills
Direct experience supervising front line staff in a health care setting
Experience working with frail elderly and adults with special needs
Well developed computer and keyboarding skills and familiarity with electronic database systems, including case notes documentation (Knowledge of AlayaCare would be an asset)
A valid Ontario driver’s license, vehicle insurance and access to a vehicle
Current CPR/First Aid Training and Vulnerability Check
