South Bruce Grey Health Centre is currently accepting applications for a Full TimeHealth System Coordinator to join our team.
POSITION SUMMARY
Reporting to the Vice President of Patient Care/CNE the Health System Coordinator plays a key role between South Bruce Grey Health Centre (SBGHC) staff, physicians and external partners in ensuring seamless, quality and efficient Health System flow activities for the SBGHC. The coordinator responds to inquiries from key stakeholders directly and/or indirectly and facilitates organizational system flow for the coordination and organization of admissions and repatriations to optimize coordinated access to health care services. This coordinator position supports professional practice with education, which includes the electronic medical record (EMR) as well as the learning management systems (LMS) platform.
The coordinator problem solves barriers to transitions in care by working closely with the Home and Community Care Team, community partners, information technology, clinical managers, and the unit charge nurses/TL.
The coordinator completes daily updates to the SBGHC bed board, and provides daily, weekly, and monthly reports that are provided to relevant leaders in the hospital and is fully informed of issues and challenges that require action.
POSITION QUALIFICATIONS & PREREQUISITES:
- Current registration in good standing with the College of Nurses of Ontario
- Experience with Electronic medical record systems
- Minimum of 3 years of previous hospital experience in direct patient care
- Knowledge of clinical applications including Cerner Millennium is preferred
- Demonstrated interest in the use of clinical applications and a desire to mentor others an asset
- Project management experience in health care an asset
- Skilled in computer applications including Office 365, Word, Excel, PowerPoint, Outlook and internet-based applications
- Ability to develop and present reports and presentations, and draft correspondence; strong presentation skills
- Respect for confidentiality and knowledge of all relevant privacy legislation, complying to
- PIPEDA/PHIPA
- Demonstrated ability to establish and maintain positive working relationships with others both internally and externally
- Demonstrated interpersonal, written and verbal communication skills and ability to demonstrate tact and diplomacy with diverse groups of stakeholders
- Effective critical thinking, troubleshooting, problem-solving, decision-making, and organizational skills
- Commitment to continuous professional development and continuing education
- Available for regular assignment of Hospital Administrator On-Call responsibilities (assigned on a rotating basis to cover evenings and weekends)
- Ability to travel to other sites as required
- Adherence to SBGHC’s core values, policies and procedures, Occupational Health & Safety and Infection Control practices and principles
