Responsibilities:
- Provide case management services to clients, including assessment, planning, and coordination of services
- Conduct interviews and assessments to determine client needs and eligibility for programs
- Develop and implement individualized service plans for clients
- Monitor client progress and make necessary adjustments to service plans
- Collaborate with other professionals and agencies to ensure comprehensive care for clients
- Maintain accurate and up-to-date client records and documentation
- Assist clients in accessing community resources and support services
- Advocate for clients' rights and needs
Qualifications:
- Previous experience in case management or a related field preferred (Atleast 1-2 Years)
- Knowledge of social service programs and resources
- Strong communication skills, both written and verbal
- Proficiency in Microsoft Office Suite (Word, Outlook)
- Excellent organizational and time management skills
- Ability to work independently and as part of a team
- Strong problem-solving skills
- Ability to maintain confidentiality
