Responsibilities
- Ability to work as part of a multi-disciplinary team with community agencies, professionals and specialists.
- Responsible for planning, developing, implementing and monitoring Individual Support Plans in such areas as: daily living skills, social maturity, communication skills, behaviour, recreation and leisure skills, as well as developing baselines, programs, care plans and such client related meetings.
- Responsible for assessment, case conference and progress reports as well as attending programming related meetings.
- In conjunction with the Service Coordinator identify and develop behavioural support plans, treatment plans and the monitoring and maintenance of such.
- Orientate team members in areas pertaining to client needs, programs, medical history, medication delivery and client program record keeping.
- To monitor client service delivery to ensure it keeps within parameters as determined by client rights, Abuse Reporting Protocol, related policies and management practices.
- On-call responsibilities and available to work flexible shifts if needed.
Qualifications
- Post-secondary qualification in Rehabilitation Studies or related area of study and/or related work experience. Behaviour management knowledge would be an asset.
Requirements
- A valid driver’s license, vehicle with a minimum of $1,000,000 insurance coverage (to be able to transport clients).
- Good written and verbal communication skills and sound decision making is required.
- The ability to work well as a team member
- Ability to use Office 365.
- Police Clearance (Vulnerable Sector Check, six months or newer and clear).
- This position will be 40 hours per week in our Community Access Program.
