Lynwood Charlton Centre (LCC) is a publicly funded charitable organization in the City of Hamilton which provides a spectrum of innovative and evidence-based, mental health services, including congregate care, day treatment, community-based services including in–home and school-based services, to children, youth, families and the community.
The Centre is funded and licensed by the Ministry for Children and Youth Services and accredited by the Canadian Centre for Accreditation. We are currently looking for Full Time Home Resource Worker to join our Guiding Partnerships and Solutions (GPS) Home Program on Contract.
Role Overview
Working under the direction of the Manager of Community Based Programs, the Home Resource Worker is responsibility for provision of evidence informed treatment interventions and mental health services to children/ youth and families in in their schools, homes, and community. The GPS Home Resource worker will perform case management duties through developing effective relationships with service providers in the community.

Main Duties and Responsibilities
· Provide direct service/counselling to the child / youth and or family in their home, school and/or community.
· Employ a systemic approach to develop a strengths-based treatment plan which provides a wide range of evidence informed treatment interventions appropriate to the needs and strengths of the child/youth and family by:
o Utilizing relevant intake and service history
o accessing both formal and informal resources
o utilization of standardized assessment recommendations
o regular supervision
o access to consultation
· Provision of information and education to children/youth, families and community collaterals to support the implementation of the treatment plan
· Maintain clinical records, assessments and reports according to agency and program policy and procedures.
· Share responsibility to be on-call 24 hours per day to respond to children/youth and families who are experiencing crisis
· May facilitate group work.
· Contribute to the overall case planning as generated by the child/youth, families and the interdisciplinary team.
· Utilize referral information and explore the child/youth and families’ previous involvement in community services to inform case planning
· Support and provide advocacy for the child/youth and family in accessing and/or making referrals to community resources.
· Liaise, facilitate and participate in meetings with community partners.
· Attend and participate in team meetings including case consultations
· Contribute to the development, implementation and evaluation of the Intensive Child & Family Services Program.
· Contribute to effective team development and processes to promote team cohesion and communication
· May be required to contribute to the development of the field by supervising a student on placement.
· Participate in all mandatory agency and program training and in-service processes.
· Assume responsibility for their own professional development, including keeping up to date with new developments in the field and acquiring new skills to best serve the children/youth and families within the Program.

Skills Required
· Assess client strengths, needs and resources through effective interviewing and counselling skills
· Read and integrate information from referral information, reports and assessments and case histories
· Apply recommendations from standardized clinical assessments to develop treatment plans
· Employ, model and teach appropriate child/youth behaviour management strategies
· Evaluate the effectiveness of treatment plans
· Apply clinical resources and Evidence Informed Practices and incorporate clinical training in day to day work
· Utilize effective crisis management strategies in various formats including telephone calls, in person, and debriefing situations
· Ability to implement all strategies as outlined in the Safe Management Group (SMG) Crisis Intervention program including use of physical intervention.
· Identify risks and problems, seek and consider alternative solutions
· Separate non-essential from critical data
· Make decisions within the scope of practice and consistent with the program guidelines, ethical framework, policies and procedures of the agency
· Develop rapport and relationships with children/ youth and families
· Work collaboratively and cooperatively with internal and external parties
· Utilize effective conflict management strategies in navigating contentious issues.
· Prioritize duties and tasks and utilize computer software to schedule work/appointments independently
· Meet deadlines and accomplish assigned volume of work within allocated work time
· Utilize computer databases, forms and templates to document client service interactions and service reporting
· Complete all documentation in an organized, logical, easily understood, concise manner, that is suitable to the audience
· Demonstrate personal and professional boundaries and maintain the privacy and confidentiality of client information
· Employ an ethical framework as prescribed by policy and procedures
· Represent the program and agency both internally and externally reflecting agency values and core principles
· Present information to various internal and external audiences in an organized, logical, easily understood, concise manner, that is suitable to the format and audience
Experience Required
· Several years of progressive experience working with families and children and/or youth in a treatment/counselling setting.
· Experience working with and developing intervention plans aimed to enhance client functioning
· Experience working with families and teaching parenting strategies
· Experience collaborating with an interdisciplinary team to set and achieve common goals.
· Experience with navigating and/or collaborating with other community systems
Knowledge Required
· Knowledge of child and youth mental health and relevant evidence informed treatment interventions
Education and Training Qualification(s)
· Diploma in CYC or Equivalent from recognized community college or a BA in a related field
· Certification in CHYMHs assessment or certification obtained within 3 months of hire
· Certification in SMG’s CI Training
· First Aid Certificate in good standing
