Belleville & Quinte West Community Health Centre (BQWCHC) is a community-based primary health care organization that provides comprehensive primary health care services, community-focused health promotion and development. We work from a health equity perspective prioritizing people and communities who face systemic barriers to accessing services. The Community Resource Worker works with BQWCHC clients and community members to facilitate access and coordination of the various agencies/providers/services that are involved in the care of clients/community members with unmet health and social needs. The Community Resource Worker (CRW) will provide time-limited support in the navigation of the health and social services system and development of sustainable strategies for the mitigation of barriers to access to more optimal health and wellbeing in alignment with client-stated goals. The CRW works as part of a team (including the QW Community Resource Worker and the Supportive Housing Case Manager) in collaboration with the BQWCHC interdisciplinary team to support the client.
Key Areas of Responsibility:
- Work with client and client’s care team (internal and external) to clearly identify client-stated goals
- Liaise with client and care team to ensure all available resources are identified and engaged to advance stated goals
- Ensure seamless up-to-date communication and documentation among coordinating partners
- Connect with clients by telephone, in-person at the BQWCHC locations, in the community and through home visits as needed to address the needs of the client/caregiver(s)
- Work in a manner that incorporates health promotion and recognizes the social determinants of health
- Maintain current knowledge of resources available in the community, foster ongoing partnerships with Community Resources and establish new community partnerships, as appropriate
Qualifications:
- Diploma in social services or related field with several years of experience or an equivalent combination of experience and education
- Minimum three experience in a community setting; experience in a primary care setting (collaborative practice/team-based approach to care), an asset
- Demonstrated understanding of the social determinants of health and client/person-centered approach to care, including advocacy, networking and navigation skills
- Experience working with and understanding of issues facing individuals and families living on low incomes and/or facing barriers to access
- Demonstrated knowledge of Community Resources and the strengths & challenges of the Belleville and Quinte West communities
- Proven ability to obtain and maintain trust with community members & agencies
- Excellent interpersonal and client advocacy skills; verbal and written communication skills (interviewing, counselling and facilitation skills)
- Basic computer proficiency in MS Office required; proficiency with EMR is an asset
- A valid driver’s license, reliable vehicle and willingness to travel within the Quinte area is required
