Job Purpose/Summary:
The Intake Coordinator is a key member of the Hospice Team and is the point of contact for clients requesting hospice services including PCCT, Grief Programing and the Residence. The Intake
Coordinator will review and prioritizes all referrals, enters clients into the database, collects additional information as needed and connects the client with other team members including booking appointments. The Intake Coordinator ensures client referrals are managed in a timely, accurate and sensitive manner.
Primary Duties and Responsibilities:
- As the ongoing in-the-office point of contact for clients and community partners the Intake
- Coordinator will: o Ensure referrals are complete, triaged and connected with the appropriate staff and/or service within Hospice. o Connect with clients and caregivers to ensure professional and timely follow up to the referral and check in with them as needed to ensure consistent care. o Book client appointments and visits and ensure chart and electronic record is ready for staff to enter notes. o Maintain regular and timely documentation and statistical records. o Manage client files, ensuring confidentiality and appropriateness of information. o Connect with physicians, and community partners including the Palliative Care
Unit at PRHC and Home and Community Care Services to ensure seamless care for clients. o Maintain information in client databases, this may include learning to pull statistics from the databases.
- Provide information on Hospice care and services when people contact the organization.

Organizational Responsibilities:
- Represent Hospice Peterborough in selected community activities, education opportunities and committees.
- Participate in assigned Hospice activities and committees.
- Participate in the completion of administrative reports on a timely basis including: o Statistical collection for reporting. o Ensure accuracy of performance indicators. o Ensure robustness of data collection.
- Participate with the executive director in an annual performance appraisal.
- Fulfill all responsibilities in accordance with the by-laws, policies and procedures and standards of Hospice Peterborough.
- Willingness to accept other duties as required.

Qualifications:
- Undergraduate degree or diploma is required.
- Additional education or certification in supporting clients through hospice palliative care experience including grief and bereavement is required.
- Demonstrated written and oral communication skills including ability to interact with clients over the phone, in person or via email required.
- Demonstrated computer skills required (Word, Excel, Outlook, etc) required.
- Demonstrated database management and statistical reporting skills required.

- Commitment to a hospice palliative care approach, and an understanding of family dynamics in illness and grief
- Ability to demonstrate compassion, empathy, integrity, respect, courtesy and a caring attitude in all interactions.
- Outstanding organizational skills and ability to multitask is required
- Knowledge of community health and social service resources an asset.
- Work well in a team environment.
- Current and original copy of a satisfactory Criminal Records Check, Vulnerable Sector Search and Child Abuse Registry Check is required upon the start of your employment.
