We are currently seeking a dedicated and experienced Intake Coordinator to join our team on a casual basis. This role involves fieldwork, assessing potential clients, and conducting check-ups or re-assessments for current clients.

Position: Casual Intake Coordinator

Responsibilities:

• Conduct field assessments for new potential clients.
• Perform check-ups or re-assessments for current clients.
• Collaborate with the healthcare team to ensure comprehensive care plans.
• Maintain accurate and up-to-date client records.
• Participate in team meetings to discuss and review cases.
• Provide compassionate and patient-centered care.

Qualifications:

• Registered Nurse (RN) with a valid license in Ontario.
• Proven experience in community health, home care, or a similar role.
• Strong assessment and communication skills.
• Ability to work independently and collaboratively within a team.
• Proficient in relevant computer applications for documentation.

Requirements:

• Availability for flexible working hours and occasional weekends.
• Access to reliable transportation for field visits.
• Commitment to maintaining confidentiality and professionalism.
• Willingness to perform administrative tasks as needed.
