We have an amazing group of staff providing support for our clients and their choices, throughout our various programs and locations across the Grey-Bruce region.

We are currently seeking 1 part-time Personal Support Worker to join our wonderful team. This position is for our Intensive Community Supports Program where you will be working one on one with the supported individual in the client’s home, this position is for up to 24hrs/week.

What the position entails:

• Work with the family in providing exceptional person-centered supports
• Provide a wide range of support for individuals following Individualized Support Plans (ISP)
• Assess clients' needs and encourage healthy life decisions
• Administering and documenting medications
• Monitoring the health and wellness of the client
• Facilitating communication between client and their medical support team with the assistance of the family
• Assisting with ordering and organizing medical support needs
• Collaborate with other services to provide dynamic and individualized Community Supports
• Assist with day-to-day needs
• Supporting the client with personal care needs
• Support physical and communication developments, or other goals set by the individuals

Qualifications:

• Completion of relevant college-level PSW training.
• Professional or personal experience working with individuals who have complex needs
• Ability to work independently and in collaboration with the family
• Valid CPR and First Aid certification
• A clean driver's abstract to be added to company insurance, if use of our vehicles is required
• Leadership experience considered an asset
• Demonstrated sound and critical thinking skills
• Knowledge or experience within palliative care
