We are seeking a passionate, energetic, and resident-focused RAI Coordinator to join our management team at Centennial Place LTC. The successful candidate demonstrates a commitment to excellence in care services and leadership in creating a positive and inclusive workplace.
The RAI Coordinator ensures that all Resident Assessment Instrument-Minimum Data Set 2.0 (RAI-MDS) processes are in operational compliance with MLTC requirements and objectives. Reporting to the Director of Care, you will lead our interdisciplinary team in achieving RAI MDS objectives.
As our RAI Coordinator, you will:
- Lead the Home’s RAI-MDS processes in accordance with MOHLTC guidelines and the Home’s policies
- Coordinate and complete RAI-MDS resident assessments, providing leadership to staff in assessment processes within the Home
- Provide RAI-MDS education through group education sessions and individualized coaching to all RAI users
- Actively monitor RAI users and assessments for quality and accuracy of data, implementing quality improvement measures when needed
- Provide critical analysis of all data levels and quality indicators, providing data interpretation and reports to management
- Review resident referrals and ensure residents are assessed according to RAI-MDS requirements and the Home’s strategic objectives
- Demonstrates compassion and professionalism in interactions with residents and their families at all times
- Promotes teamwork and good relationships with co-workers, assisting as required and demonstrating dependability
- May be assigned other nursing and supervisory duties based on the needs of the Home
Skills and Qualifications Required:
- Current registration as a member of the College of Nurses of Ontario
- Passion for working with older adults and the ability to lead a team
- 1 to 3 years of experience in a Long-Term Care setting
- Good working knowledge o f the LTCHA regulations and MLTC funding mechanisms
- Excellent communication and interpersonal skills
- Organized with excellent time-management skills
