Job description
The role of Community Support Navigator is to facilitate the care of patients by conducting screenings and initial psycho/social assessments and assisting patients in navigating and accessing appropriate community-based health services/programs. Additionally, support patient care with appropriate follow-up, and at times by providing interim care, or assessment for psycho-educational groups.
Functioning as part of a multidisciplinary team, Social Workers work with PCN physicians, other health professionals, and local and regional programs to ensure a high quality of evidence-based health care is delivered to the population we serve.
Being industrious, enthusiastic, and adaptable are essential qualities for Kalyna Country PCN Social Workers as they are expected to contribute to the development and implementation of novel, integrated, patient-focused, community-based solutions to mental health services.
Work Experience or Education
· A Social Worker diploma or Bachelor's Degree in a health discipline is an asset or diploma in related area of work
· Current registration with the appropriate professional organization.
· At least 3+ years of work experience in supervisory in health care delivery.
· Experience working in a team setting with other health professionals.
· Basic computer skills, ability to use Electronic Medical Records
· Strong, effective written and oral communication skills, coupled with a high level of perceptiveness.
· Ability to work proactively, effectively, and independently in a complex changing environment.
· High levels of integrity and professional behavior.
· Clear criminal record check
· CPR
· Valid drivers license
No relocation funding available
This is a temporary maternity coverage Starting April 30,2024- April 2025
Hours are set at an 0.6 FTE
The role will require working within the PCN office & community clinics in Tofield & Vegreville in person. Virtually for Vermilion.
