The Parry Sound Friendship Centre Rural-Urban is currently accepting resumes for a full-time “Life Long Care Coordinator”. This position will be responsible for the development of community support services with particular attention to the needs of Indigenous seniors/elders, the “disabled” and the chronically ill with special needs and to provide for the development of a comprehensive volunteer support circle. This position reports to the “Life Promotion Manager-Youth/Adult”.
RESPONSIBILITIES
The Life Long Care Coordinator will be responsible for the development of Community Support Services base on the community needs, such as, but not limited to:
· Meal programs
· Transportation to medical appointments
· Friendly visits
· Care-giver support
· Adult day programs
· Respite care
· Shall be responsible to provide support to the development of a comprehensive volunteer support circle
· Will also ensure the development of community support services with particular attention to the needs of indigenous seniors/elders, those individuals with disabilities and the chronic ill with special needs.
Qualifications
· Diploma in related field or equivalent work experience
· Indigenous teachings and live experience with cultural traditions and ceremony
· Knowledge and experience in the area of long term care for Indigenous people in Parry Sound-Muskoka and surrounding area
· Experience in group facilitation and programming
· Ability to assess participants needs
· Knowledge of social barriers and challenges faced by Urban Indigenous people
· Excellent communication skills, both verbal and written
· Excellent computer skills and database reporting both qualitive and quantitative
· Strong problem-solving skills and ability to make sound decisions
· Time management skills with the ability to prioritize a high volume of work
· Ability to understand and follow oral and written instructions and to complete assigned tasks with minimal direction
· Excellent program management and planning skills and the ability to successfully multitask
· Highly skilled in developing relationships with agencies and co-workers
· Experience working within an Indigenous community-based organization
· Must have a valid driver’s license and reliable vehicle
· Satisfactory Level 3 vulnerable sector criminal records check
· Experience in growth based organizations
