Job description

The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & StonyPoint, Watford, and Sarnia. Our interdisciplinary team of providers includes: physicians, nurse practitioners, registered nurses, social workers, health promoters, dietitians and physiotherapists.


Full Time Program Assistant - Hep C/ Harm Reduction

North Lambton Community Health Centre is looking for a Program Assistant to support the team and clients. As a member of the Hep C/ Harm Reduction team, the primary role of the Program Assistant to enhance community coordination of services that affect people living with / at risk of acquiring hepatitis C, and other at risk populations. The successful candidate will provide direct administrative support in collaboration with the team, and participate in program delivery related to Hep C and Harm Reduction. Responsibilities will include:


Team Coordination

  • Liaising with appropriate community resources on behalf of clients
  • Maintaining referral and follow up procedures/inboxes
  • Coordinate information resources on health care alternatives, Harm Reduction and care options for clients affected by HCV
  • Identify emerging issues in HCV education and prevention through researching articles, books and other materials as well as liaising with other agencies

Program Assistance and General Administration

  • Assist with existing programs (ie. methadone clinic)
  • Preparing and editing presentations to deliver to community partners
  • Design and develop training materials to ensure that the goals of the program are being achieved and the needs of those requesting raining are being met
  • Routinely compile, enter and report confidential data at agency and funders' request
  • Ensure that all necessary repots are completed and submitted to the appropriate sources meeting set deadlines
  • Participate in the planning of program evaluation initiatives
  • Participate as a team member in all team functions - program planning, team meetings, inter0team meetings, record keeping, minutes and follow up
  • Network with internal and community partners to ensure an optimal continuum of health and social services through all stages of HCV education, support, care, and treatment
  • Maintaining and develop professional competence and learning
  • Other related duties as assigned

Qualifications:

  • Two (2) years’ experience working within a community setting
  • Knowledge of Hepatitis C and Harm Reduction
  • Strong knowledge of issues affecting marginalized communities
  • Experience working with diverse agencies and working successfully in partnership with these agencies
  • Thorough knowledge and understanding of the local health and social service sector, government programs and current legislation that may affect clients
  • Demonstrated ability to work under pressure, to anticipate potential problems/conflicts and take appropriate actions to meet deadlines
  • Excellent interpersonal, presentation and communication skills
  • Ability to work independently and as a team member within an evolving role in a multidisciplinary environment
  • Excellent knowledge of MS computer applications and other office related software.
