Job Description

The Project Coordinator, SCOPE is responsible for supporting the local and regional coordination, implementation and growth of the Seamless Care Optimizing the Patient Experience (SCOPE) program. SCOPE is a virtual interprofessional health team that supports primary care providers through a single point of access to local specialists, imaging, and community services, to serve their patients with complex care needs. Leadership for the SCOPE program is provided jointly by the University Health Network and Women's College Hospital.

The Project Coordinator builds and fosters strong working relationships with key stakeholders including Primary Care Providers. They will provide leadership and subject matter expertise in the following areas:
  • Facilitate the recruitment and onboarding of new community physicians;
  • Maintain an up-to-date tracking database and provide regular summaries to the SCOPE team regarding ongoing use, feedback, trends and other relevant data;
  • Develop quarterly scorecards, yearly reports, and manage ad hoc queries as required by key stakeholders;
  • Support the SCOPE program in coordinating physician engagement activities, including the development of electronic communications;
  • Coordinate the overall evaluation of SCOPE, including but not limited to:
o Ensuring timely and effective communication among project leaders by drafting correspondence and scheduling meetings;
o Preparing and maintaining research ethics board applications;
o Assisting with the collection of data for the SCOPE economic analysis;
o Developing, distributing, and reporting of physician and patient surveys;
o Collaborate with the WIHV evaluation team to develop protocols for sub-studies;
  • Draft conference abstracts and manuscripts; Assist Scientists in drafting peer reviewed journal articles for publication;
  • Collaborate with team members to write and edit presentations, funding proposals, and grant applications;
  • Liaise with WCH healthcare providers in furtherance of project objectives.
As a role model and champion, you will work to identify and integrate safe, best practices into daily activities to foster the delivery of safe and exemplary care.

Qualifications
  • Undergraduate degree in project management, business management or a related field
  • Minimum of 3 years of project coordination experience in a similar role.
  • Experience preparing, interpreting and scheduling implementation plans with project management tools and concepts.
  • Experience with database applications including REDCAP.
  • Experience with multi-stakeholder communications and ability to maintain relationships.
  • Project management concepts and knowledge.
  • Strong organizational and time management skills.
  • Strong interpersonal and communication (oral & written) skills.
  • Ability to effectively prioritize and execute tasks.
  • Problem solving skills and ability to analyze workflow or processes.
  • Accounting and bookkeeping skills.
  • Research and analytical skills.
  • Good work and attendance record required
