This position is responsible for assisting and supporting the clients in their daily routines and ensuring clients receive quality care; and that service delivery is carried out in accordance with the Excel Society’s Philosophy, Policies, and Operational Procedures.
Specific Responsibilities:
1. Adhering to all Excel Policies and Procedures at all times.
2. To actively participate with Excel Society’s Health and Safety Program; including but not limited to: participating with hazard identification and assessments; worksite inspections and reporting noted safety concerns; and participating with hazard control efforts.
3. To be aware of and implement as required Excel Society’s Safety Emergency Response Procedures.
4. Make timely and informed decisions that consider facts, goals, constraints, risk, Excel Society Values, and the Excel Advantage.
5. Providing quality care and professional direct client support in a manner which reflects the values of the Excel Society and honours the “Client’s Rights” statement.
6. Contributing to assessing client(s)’ service needs and/or accompanying client(s)’ to available relevant assessment services.
7. Utilizing appropriate data collection methods to collect information regarding behaviours of concern or to teach new skills.
8. Ensuring that health and medical needs of the clients are met.
9. Contributing to the investigation of available community leisure, recreational resources, vocational opportunities, educational options and any other Community Support options identified by the client and/or their support circle.
10. Participating fully in developing, maintaining, cleaning, and utilizing the program’s physical environment and resources
11. Assisting clients with their financial support needs.
12. Advocating for the needs of each client and balancing the needs of each individual living at the community residence, within their team or work site.
13. Working in accordance with contracts and/or work schedules posted, unless otherwise approved by the immediate supervisor or their designate
14. Implementing all aspects of daily living skill ensuring client(s)’ needs are met as designated.
15. Supporting clients other than those previously identified at the community residence in a professional individualized manner consistent with each client’s service needs.
16. Being a supporting, positive, and professional team member.
17. Contributing as a part of the team to the development of an Individual Support Plan, Essential Lifestyle Plan, or Person Centered Plan (as appropriate) for each client they directly support.
18. Ensuring that all pertinent information is exchanged between staff members during shift exchanges.
19. Follow all planned positive and restrictive procedure as well as utilize skills taught in training regarding behaviour and mental health supports and follow policy and procedures regarding unanticipated behaviours.
20. Attending and actively participating in staff “team meetings” and in staff development programs as required.
21. Liaising with families, guardians, professionals and other resource members of the client(s)’ support network, communicating relevant client information to other team members.
22. Regularly read and respond to house emails and Excel Society assigned emails for effective communication.
23. Being aware of the roles and follow identified responsibility expectations of the Emergency Response Lifeline Call System.
Requirements:
1. A certificate in disability studies and/or social work from a recognized institution. Equivalencies may be considered.
2. Maintain certification in First Aid and CPR.
3. Satisfactory criminal record check.
4. Knowledgeable and experienced with various diagnosis surrounding persons with disabilities.
5. Good and effective planning, organizational, communication, and people skills.
6. Ability to work independently and in a team environment.
7. Possess well developed computer use skills.
